All information requested on this form must be completed and accompanying documentation or sources listed as well as letters of recommendation received on or before January 15, 2023 for consideration.

All information submitted will be retained by the Cranston Athletic HOF selection committee. The completed application packet must include:

2.)  Supporting materials such as newspaper clippings, magazine articles and other supportive materials.

3.)  A photograph for possible publicity purposes may accompany this nomination.
